All benefits are available to eligible & full-time government officers and employees who are actively at work on the effective date of the group insurance and also are extended to cover the insured’s legal spouse.
All government officers, employees and their spouse (eligible members) must attain age of 15 – 59 years old.
All benefits are available to the eligible member with risk exposure not higher than the occupational class 2. (White & Light - Blue Collars only)
The premium rate is based on monthly basis and the attained age of each insured at each policy anniversary.
The premium payment will be deducted from the payroll account of each insured government officer or employee with their consent.
Since the insured member is allowed to pay the premium on monthly basis, he/she is required to pay the premium for every consecutive month. In case of premium lapse during the policy year, the insurance coverage of the insured member (including his spouse) will be terminated on the last day of the month, which the premium has been paid.
The minimum number of participating members (excluding spouse) is 50.
All eligible members (including their spouse) are required to complete the Health Declaration Form.
The enrollment of the new eligible members and their legal spouse is allowed at the policy anniversary only.
In case that an insured enrolled and has been currently covered under this group insurance program, he would not be allowed to re-enroll.
The eligible member is required to choose either one of insurance plans, and his spouse is required to enroll the same plan as the insured.
The effective date will be the first day of the following month after the insurability is approved and the premium is paid.
Since this insurance program is on voluntary basis, the insurance coverage will be provided to the eligible member until the last day of the month, which the premium has been paid. However, the certificate will be issued to each insured member as an evidence.
In the event of termination of employment, the insurance coverage of both insured and his spouse is allowed to be in force until the last day of the month, which the premium has been paid.
The eligible member and his spouse (if applicable) are required to complete the Health Declaration Form with specifying the chosen insurance plan together with their ID Card photocopies and then pass to your “Human Resource Department”. AIA will inform the effective date to the insured member afterwards.
In case that an insured enrolled and has been currently covered under this group insurance program, he would not be required to re-enroll at the renewal.
The Master Application Form completed by the employer
The Health Declaration Form completed by each insured member and spouse (if applicable)
ID Card Photocopies of each insured member and spouse
A data sheet containing a detail summary of all insured members and spouse
An evidence to prove the marital status (either one photocopy of marital certificate, birth certificate of child, or residential certificate)
Premium payment by cheque to American International Assurance Company, Limited
This insurance program is not allowed to sell in high-risk areas including Chiangmai, Chiangrai, Lumpang, Lumpoon, Maehongsorn, Payao, Narn, Prae and Rayong.
Since this insurance program is on Yearly Renewal Term basis, AIA reserves the right to change the terms and provisions as well as premium rats and policy cancellation within the 31 days prior notice before the policy anniversary.
