Group Hospital
Income Benefit
.........Group
Hospital Income Benefit is a Supplementary Contract attached
to and forming a part of Group Life Insurance Policy (Principle
Policy). This Supplementary Contract will provide for an Insured
Member the reimbursement of medical expense incurred daily when
he is admitted into the hospital as a result of sickness or
accidental injury.
Coverage
.........The
benefit covers for 24 hours in and out of occupation without
geographical limit provided that the Insured Member has stayed
in hospital for at least 6 consecutive hours for treatment of
sickness or injury.
Benefits
The benefit is payable at a rate per day subject to the daily
maximum benefit stated in the Schedule of this Supplementary
Contract.
Exclusion
The
company shall not pay the benefit under this Supplementary Contract
for the following occurrences
- Pre-existing
conditions for which the Insured Member received medical
treatment within 90 days preceding the effective date of
his coverage unless the Insured Member has been insured
under this Supplementary Contract for least 12 consecutive
months.
- Self-destruction or intentional
self-inflicted Injuries due to functional disorders of the
mind including anxiety state, depression, neuroses, psychosis,
neurasthenia; drug addiction or alcoholism.
- Injuries arising from wars, revolutions
or any warlike operations.
- Riot and civil commotion, strikes
or terrorist activities.
- Special nursing care; general physical
check-up or any treatment which is not medically necessary;
dental treatment or surgery except that such procedures
are necessitated as a result of accidental injuries.
- Congenital anomalies; treatment
occasioned by or resulting from pregnancy, childbirth, miscarriage
or abortion or relating to birth control, sterilization
of either sexs, treatments pertaining to infertility.
- Cosmetic treatment or surgery for
purposes of beautifications; non-medical personal services
such as radio, telephone and the like; procurement or use
of special braces, appliances or equipments.
